The HUB is an evidence-based model of community care coordination that focuses on addressing social determinants of health (SDOH) risk factors in food insecure or other high-risk individuals. The HUB provides the structure to ensure that Winona’s most vulnerable families’ complex needs are addressed in an equitable, unduplicated, and meaningful way. The target population for referrals is:
Winona County Residents and
- Households with minors (17 years old and under) OR
- Family is either uninsured or insured through Minnesota Medicaid OR
- Someone in the household has screened as food insecure OR
- Diagnosed or self-reported mental health concerns OR
- Is experiencing homelessness.

La Crosse, WI
Family and Children's Center is a Care Coordination Agency with Great Rivers HUB in La Crosse County. Our Community Health Workers help individuals and families identify their risks and outcomes to achieve (pathways) that will help reduce their risk. This occurs while navigating the family through the systems to obtain the services needed. By participating in the HUB, services for families are streamlined, resulting in better communications and more effective care.
