Great Rivers United Way Community Care Program

Family & Children’s Center is a Care Coordination Agency with Great Rivers United Way Community Care Program in La Crosse County. Our Community Health Workers help individuals and families identify their risks and outcomes to achieve (pathways) that will help reduce their risk. This occurs while navigating the family through the systems to obtain the services needed. By participating in the Great Rivers United Way Community Care Program, services for families are streamlined, resulting in better communications and more effective care.
